if you want haggle over it you could say just a dropmore light between head and raised up arm would be perfect !
professional device + studio give nevertheless an advantage !

I use 2 flash for this shot Hans. U can see the 2nd flash highlighting the left hand & torso of the model.

just needs a little airbrushing at armpit
select the eyedropper tool on the darker colour and get a soft brush and bobs your uncle sucess
fine lighting
